This dreadful boss shares his loot pool with Andariel and the Harbinger of Hatred, who are therefore just as likely to drop this unique weapon, although Duriel remains the least random source.
You will need to fight Duriel on Torment 1 minimum, but to access his treasure, you will need three Agony Fragments, a traditional item that can only be collected from Varshan near the Tree of Whispers, or from Grigoire south of Ked Bardu.
Belial can alternatively let you obtain items from any stronghold boss loot pool, which makes him an interesting alternative.
